
Spartanburg, South Carolina Oct 15, 2025 (Issuewire.com) - South Carolina native and community strategist Jasmine Guest-Sanders has announced the launch of Rise SC, a new statewide initiative designed to connect families with resources, clinics, and community support for chronic and genetic diseases beginning with sickle cell disease (SCD).
Born in Orangeburg, raised and educated in the Pee Dee, and now based in Spartanburg, Guest-Sanders created Rise SC to bridge the information gaps she witnessed firsthand across rural and underserved communities in South Carolina.
Ive seen families struggle to find consistent, clear information about where to turn for care, Guest-Sanders said. For too long, access to resources for conditions like sickle cell has depended on who you know or how far you can travel. Rise SC was built to change that to make finding help simple, local, and equitable.
A Platform Rooted in Access and Equity
Rise SC is a digital resource hub designed to map chronic-disease services and supports across
South Carolina, highlighting:
-Tier 1: Specialized care (sickle-cell and hematology clinics)
-Tier 2: Federally Qualified Health Centers (FQHCs), primary care, and community support
programs
-Community Resources: Mental health, daily living, school, and transition tools for families
The goal: make navigation easier for minority and rural families facing long travel times, limited
specialty providers, and fragmented information.
South Carolina has an incredible network of hospitals, nonprofits, and advocates doing this work but families shouldnt have to search a dozen sites or drive across counties to find help, Guest-Sanders said. Rise SC connects those dots.
Rise SC will continue expanding beyond sickle cell disease to include other chronic conditions that disproportionately impact minority communities, such as lupus, sarcoidosis, and reproductive-health disorders. Guest-Sanders invites collaboration with health agencies, researchers, and community organizations to strengthen the hubs reach and accuracy.
